- Test Permit (should be printed)
- Identification Card (School ID)
- For non-SHS graduate and SHS graduate in the previous school years, please do not forget to bring and present your ORIGINAL COPY OF FORM 138 (REPORT CARD) during your test schedule.
TESTING ASSEMBLY AREA: 1st Floor, Student Center Building, Lucinda Extension Campus
Note: Please come at least 30 minutes before your scheduled time of examination. Late examinees will be rescheduled on a later date.
